29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence: a. On or about June 2018, an employee was exposed to machine hazards associated with pinch point and point of operation hazards during servicing, maintenance, and setting up the Dunmar Brake Press machine. The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910.147(d)(4) requirements]. As a result, the remaining applicable energy control elements, involving dissipation of residual energy [(d)(5)(I)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On or about June 18, 2018, employees were operating the Durma Brake Press and two belt sanders, and the equipment was not guarded to protect employees, thereby employees were exposed to point of operation, ingoing nip point, and rotating parts hazards.

29 CFR 1910.37(b)(6): Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color. Self-luminous or electroluminescent signs that have a minimum luminance surface value of at least .06 footlamberts (0.21 cd/m2) are permitted. a. On or about June 18, 2018, employees were working in the shop area and the exit sign was not illuminated, thereby exposing employees to hazards associated with fire or other emergencies that require immediate evacuation.
