29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered. On or about 18 September 2018, and at times prior thereto, two employees in the Pre-Price Stack-Off Room were performing work without the use of cold weather footwear, exposing the employees to the hazards of cold stress related injuries.

29 CFR 1910.133(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On or about 18 September 2018, and at times prior thereto, employees working the lines in the Specialty Department were not utilizing eye and face protection where splashes occurred from a liquid chemical mixture to the face and eyes, exposing the workers to the hazard of caustic burns to the skin and eye(s).
