29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. On or about 21 June 2018, and at times prior thereto, three roofers were performing roofing activities with work ongoing above without the use of hard hats, exposing the roofers to the hazard of being struck by falling tools, materials, or debris.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles. On or about 21 June 2018, and at times prior thereto, a roofer was using a pneumatic nail gun to apply shingles to the roof of a business under construction without the use of safety glasses, exposing the roofer to the hazard of being struck by flying projectiles or particles.

On or about 21 June 2018, and at times prior thereto, two roofers were applying shingles to the roof of a single story business under renovations without the use of fall protection, exposing the roofers to a fall hazard of approximately 8 feet to the ground below.

On or about 21 June 2018, and at times prior thereto, two roofers accessed the roof of a business under renovations without the ladder being extended at least 3 feet above the roof edge, exposing the roofers to a fall hazard of approximately 8 feet to the ground below.
