29 CFR 1910.23(c)(4): Ladders were used on surfaces which were not stable and level:    a) Worksite, 142 Montauk Hwy, Westhampton, NY 11977 - An employee was observed accessing and working from a ladder that was placed in the bed of a 1999 Dodge pickup truck (VIN No. 3B7KF23Z5XG155243) and extended to the roof edge , on or about 06/21/2018.     Note:  In addition to abatement certification the employer is required to submit abatement documentation for this item in accordance with 29 CFR 1903.19.
