29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source(s): a) PANDROL USA, LP - Line 1-2: Where it was required, an energy isolating device had not been operated in a manner that isolated the machine from its energy source. A maintenance employee was injured while he was aligning the cropping dies and adding shims to the shear blocks while the machine was ON. The employee got his hand caught in a pinch point near the point of operation when the machine shear was activated to cycle. Occurred on or about 08/10/2018.

An employee suffered a partial amputation of his left index finger while to unjamming a steel bar at the point of operation. Occurred on or about 06/19/2018. PANDROL USA, LP was previously cited for a violation of this occupational safety and health standard or its equivalent standard1910.212(a)(1), which was contained in OSHA inspection number1126874, citation number 1, item number 1 and was affirmed as a final order on 5/31/2016, with respect to a workplace located at 501 Sharptown Road, Bridgeport NJ 08014.
