29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Roof surfaces at 8970 Gale Rowe Lane, Fairhope, AL: On or about June 22, 2018, the employer did not ensure fall protection was used while installing weatherproofing material to the roof surfaces which exposed an employee to falls of approximately 8-feet.  b) Roof surfaces at 8950 Gale Rowe Lane, Fairhope, AL: On or about June 22, 2018, the employer did not ensure fall protection was used while installing weatherproofing material to the roof surfaces which exposed an employee to falls of approximately 8-feet.  Jose M Alvarenga Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1051326, citation number 1, item number 1 and was affirmed as a final order on April 30, 2015, with respect to a workplace located at 6109 Andhurst Drive, Gulf Shores, AL.  Jose M Alvarenga Ayala was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(13), which was contained in OSHA inspection number 1051326, citation number 1, item number 1 and was affirmed as a final order on January 12, 2016, with respect to a workplace located at Hoot Owl Road, Lot 6, Loxley, AL.
