29 CFR 1910.178(l)(4)(ii)(B):  Refresher training, including an evaluation of the effectiveness of that training, was not conducted as required by paragraph (l)(4)(ii) to ensure that the operator had the knowledge and skills needed to operate the powered industrial truck safely:     On or about May 11, 2018, at the loading dock, an employee was loading mail onto a trailer while operating a powered a rider jack.  The employee was exposed fall hazard when the trailer being loaded shifted causing the rider jack to become jammed between the trailer and the dock causing the operator to be thrown from the rider jack. The employer failed to ensure employees were provided refresher training on relevant topics after being involved in an accident.    The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR.178(l)(4)(I), which was contained in OSHA inspection number 960815, citation number 1, item number 1b and was affirmed as a final order on September 23, 2014, with respect to a workplace located at U.S. Postal Service, 17500 Oakwood Blvd., Allen Park Michigan 48101.

29 CFR 1904.29(b)(3): Each recordable injury or illness was not recorded on the OSHA 300 Log and 301 Incident Report within seven calendar days of receiving information that a recordable injury or illness has occurred:    The employer did not record the following workplace injury on the OSHA 300 Log within seven calendar days of learning the injury occurred.      a.  On or about May 17, 2018 a recordable injury occurred when an employee operating a dispatching machine causing the employee to suffer a strain of the left arm and the injury had not been recorded on the OSHA 300 Log.    United States Postal Service was previously cited for a violation of an equivalent Occupational Safety and Health Standard 29 CFR 1904.4(a), which was contained in OSHA inspection number 1194085, citation number 1, item number 1 and was affirmed as a final order on January 18, 2017, with respect to a workplace located at 200 W Second St., Royal Oak, Michigan, 48068.

29 CFR 1910.303(g)(1)(i)(A): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On or about June 25, 2018, on the workroom floor, located near pillar 9G, employees were exposed to electrical hazards in that the employer did not provide sufficient access and working space to a circuit breaker panel to permit the ready and safe operation and maintenance of equipment. Electrical panel was blocked by triwall.      The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.303(g)(1), which was contained in OSHA inspection number 1055863, citation number 1, item number 1 and was affirmed as a final order on June 1, 2015, with respect to a workplace located at 250 E. Blvd Drive, Flint, Michigan.
