29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: (a) Facility, 501 Saxman Road, Latrobe, PA: On or about June 20, 2018 the employer did not have an energy control program to ensure employees who serviced or maintained equipment had control procedures to isolate equipment from energy sources and had been trained on those procedures. An employee was cleaning out a rotary unit on a Finn Bark Blower that had not been isolated from the power source when the equipment was powered on amputating the employee's fingers on their left hand.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Facility, 501 Saxman Road, Latrobe, PA: On or about June 20, 2018 the employer had not developed and implemented an energy control procedure for employees to use when servicing or maintaining the Finn Bark Blower to ensure the unit is disconnected from the energy source when performing this task. An employee sustained an amputation of the fingers on their left hand when the unit was powered on while the employee was servicing the equipment.
