29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes, by covers: (a) On or about June 25, 2018, employees were exposed to injuries from tripping in or stepping into rectangular floor holes for plumbing (11x7 inches), circular floor holes for HVAC conduit (5 inch diameter), and crawlspace accesses, on the first floor. The holes had no covers. Abatement Note: Abatement certification is required for this item (See Certification of Corrective Action Worksheet).

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: (a) On or about June 25, 2018, employees were exposed to fall hazards while entering and exiting the first floor apartments. The entrances were 31.5 inches above the ground. No device to access the entries was provided.
