29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: Jobsite located at 125 Ellington Road, South Windsor, Connecticut: On, or about, June 25, 2018, two employees were replacing a shaft on an overhead conveyor. The electrical disconnect had been tagged, but each employee had not applied a lock. The employer had not developed and implemented a lockout/tagout program to be utilized by employees when performing service/maintenance on conveyor systems. Abatement Note: 29 CFR 1910.147(f)(2) contains provisions for outside contractors working at host facilities.
