29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:      a) Job site, 1100 Smallman Street, Pittsburgh, PA: On or about June 25, 2018, during construction activities, the employer did not ensure that an employee used appropriate head protection while working to load scaffold components into an aerial lift.

29 CFR 1926.451(h)(1): In addition to wearing hardhats each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, or guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects or when the objects were too large, heavy or massive to be contained or deflected by any of the above-listed measures, the employer did not place such potential falling objects away from the edge of the surface from which they could fall and did not secure those materials as necessary to prevent their falling:       a) Job site, 1100 Smallman Street, Pittsburgh, PA: On or about June 25, 2018, during construction activities, the employer did not ensure that appropriate measures were taken to prevent objects on a roof from dropping down onto or into the basket of an aerial lift below the roof line.

29 CFR 1926.501(b)(1):  Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems:      a) Job site, 1100 Smallman Street, Pittsburgh, PA: On or about June 25, 2018, during construction activities, the employer did not ensure that an employee used appropriate fall protection systems/devices while on a roof loading scaffolding components into an aerial lift where exposed to an approximate 10 1/2 foot fall hazard.
