29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c): On or about June 26, 2018, during the installation of a sanitary sewer for the construction of a residential structure near site #129 Meadow Lark Dr., Lorain, Ohio, the employer did not ensure an employee working inside a trench, at a depth of 8 ft. and near 90 degree vertical walls, was protected from cave-ins by an adequately designed protection system. Ram-Con, LLC was previously cited for a violation of this occupational safety and health standard 1926.652(a)(1), which was contained in OSHA inspection number 1269250, citation number 1, item number 2, and was affirmed as a final order on November 30, 2017, with respect to a workplace located at 2346 Columbia Road, Valley City, Ohio.
