29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Instances A: Employees were exposed to electrical shock/burns when a three prong plug was repaired with hot glue. Instance B: At the workbench lighting and receptacles, employees were exposed to electrical shock/burns from STJW 16-3 extension cords that were connected to electrical boxes by the use of clips which were not used in accordance with listing and labeling requirements.
