A GAST vacuum pump was energized by the RPT which was, in turn, energized by an orange extension cord.

29 CFR 1910.304(g)(6)(vi)(C)(8): Portable hand lamps with exposed noncurrent-carrying metal parts that could become energized were not grounded: POTTING ROOM: On or about and at times prior to July 2, 2018, metal encased portable hand lamps with two-wire STP-2 cords were in use in the fume hood. The lamps were not grounded.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: POTTING ROOM: On or about and at times prior to July 2, 2018, an extension cord with a three way tap at the distal end was used to energize a relocatable power tap (RPT) and a Cole Parmer balance. The RPT, in turn, energized a GAST vacuum pump.

29 CFR 1910.1200(f)(6): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the information specified under paragraphs (f)(6)(i) through (ii) of this section: POTTING ROOM: On or about and at times prior to July 2, 2018, unlabeled containers of hazardous chemicals were observed in two fume hoods. None of the containers was labeled with information regarding the hazards of the chemicals. The chemicals included but were not limited to corrosive two-part epoxy adhesives containing imidazole, epoxy phenol novolac, and phthalic anhydride.
