29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    On or about June 27, 2018 and times prior thereto, the employer had not developed an energy control program with machine specific isolation procedures and periodic inspections for employees required to, but not limited to, perform maintenance on the ALO, and the welders. This condition exposed employees to shock and caught in hazards in the event of an unexpected start-up.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:    On or about June 27, 2018 and times prior thereto, the knife switch disconnects for the following equipment were partially blocked and not readily accessible in the following instances:    a) The large Simonds welder located on the side wall.   b) The middle Simonds welder, or Jeremy's welder, located in the middle of the three welders along the side wall.    Employees used the welders to weld the ends of band-saw blades together. This condition exposed employees to shock hazards in the event of an emergency.

29 CFR 1910.303(e)(1)(ii):  Electrical equipment were not marked giving voltage, current, wattage or other ratings as necessary:  On or about June 27, 2018 and times prior thereto, , panel boxes were observed without voltage markings in the following instances:  Instance a: Panel boxes supplying power to the 3 Simonds welders used to join the ends of the cut band-saws along the side wall, and Instance b: Panel box supplying power to ALO cutter, labeled HIGH LEG B.  This condition exposed employees to shock hazards in the event of an emergency.
