(a) Site: In the lower service bay at the facility at 475 N. Main Street, East Longmeadow, MA 01028 on 6/27/2018, and at times prior, employees were exposed to eye hazards such as but not limited to infections caused by the use/availability of empty wash stations or eye wash stations with expired, contaminated, or empty eye wash saline solution.
