29 CFR 1910.178(l)(4)(ii)(B): Refresher training in relevant topics shall be provided to the operator when: The operator has been involved in an accident or near-miss incident; a) On or about June 19, 2018 The Hartz Mountain Corporation did not ensure that refresher training was conducted for an operator involved in accident. An employee operating a powered industrial truck damaged an overhead heater causing a propane gas leak while conducting a pick off the top of a storage shelf. The operator was permitted to continue to operate the powered industrial truck without refresher training.
