The employer did not provide proper storage for 3M half mask cartridge respirators. Employees working in the warehouse were exposed to inhalation hazards from sodium hypochlorite.

The employer did not safely maintain the electrical service panel box located in the NE portion of the warehouse near the employee entrance. Employees were exposed to energized parts of equipment due to a missing breaker cover in the panel box.

29 CFR 1910.305(j)(4)(vi): An individual disconnecting means was not provided for each motor, but a single disconnecting means may be used for a group of motors: Employees were exposed to electrical shock hazards. The employer did not provide disconnects to control the electrical service to the pump motors in the tank farm room. Employees were exposed to electrical hazards while using temporary drop cords to energize the motors.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and caught between hazards associate with missing bolts on storage racks. This was most recently documented, on or about 06/07/2018, at 1504 Roseport Road in Elwood, Kansas, where employees were engaged in loading or unloading activities in the warehouse. Employees were exposed to struck-by and caught between hazards in that the storage racks were not fully anchored to the floor with bolts. Among other methods of abatement, feasible methods would include, to anchor the racks to the floor as per the American National Standard Institute MH 16.1: 2012, section 1.4.7. Section 1.4.7 states, "All rack columns shall be anchored to the floor with anchor bolts, which shall be designed in accordance with Section 7.3, to resist all applicable forces as described in Section 2.1 or Section 2.2."
