29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained: On or about July 19, 2018 at a worksite located at 4243 Gateway Drive, Sheboygan, WI; the employer did not ensure that each employee working on the center crane in the 3500 Ton department had affixed a personal lockout or tagout device to the group lockout device when they began the work of cleaning the ceiling of the 3500 Ton department from the isolated and locked out overhead crane.

29 CFR 1910.1200(h)(3)(ii): Employee training shall include at least the physical, health, simple asphyxiation, combustible dust, and pyrophoric gas hazards, as well as hazards not otherwise classified, of the chemicals in the work area: On or about June 19, 2018 at a worksite located at 4243 Gateway Drive, Sheboygan, WI 53081; the employer did not provide training to employees on the health hazards of cadmium and inorganic arsenic. Employees were manually removing residue from the ceiling of the facility. The residue contained lead, cadmium and inorganic arsenic.
