29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:    (a) 10 Nathan Lane, Annville, PA - Employees utilized a JLG 600-S aerial lift to access the roof to install gutters on the building and were not protected/restrained inside the basket with a fall protection harness and  lanyard, exposing the employees to a possible ejection and fall hazard of approximately 21 feet 6 inches, on or about June 13, 2018.    Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
