29 CFR 1910.217(c)(1)(i): It shall be the responsibility of the employer to provide and insure the usage of "point of operation guards" or properly applied and adjusted point of operation devices on every operation performed on a mechanical power press. See Table O-10.    a.  In the Production Area on the 80 Ton Bliss Mechanical power press, Serial Number missing, a press operator was exposed to an amputation hazard due to the opening on the side metal guard exceeded the Table O-10 requirements.    b.  In the Production Area on the 125 Ton South Bend Mechanical power press, Serial Number 74180, a press operator was exposed to an amputation hazard due to the opening on the side metal guard exceeded the Table O-10 requirements.

29 CFR 1910.217(f)(2): Instruction to operators. The employer shall train and instruct the operator in the safe method of work before starting work on any operation covered by this section. The employer shall insure by adequate supervision that correct operating procedures are being followed.    The employer did not instruct the two power press operators on the safe method of work before starting work on the Bliss 80 Ton and Southbend 125 Ton Part Mechanical power press.

29 CFR 1910.147(c)(1): Energy control program. The employer shall establish a program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performs any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative.    The employer had not developed and implemented an energy control program consisting of energy control procedures, employee training, and periodic inspections.      SST Conveyor Components, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1071443, citation number 1, item number 3 and was affirmed as a final order on December 15, 2015, with respect to a workplace located at 185 Commerce Blvd, Loveland, OH 45140.
