Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.  a) Roof - Employees conducting roofing operations were observed working outside the warning line system and had donned a full body harness as part of a personal fall arrest system but did not attach to an anchor point and were exposed to an 11 foot - 7 inch fall hazard to the ground below, on or about June 28, 2018.  No abatement certification or documentation required.
