Employees were exposed to amputation injuries when performing work around the unguarded parts. b) On or about June 27, 2018, the employer did not ensure that employees were protected from the nip points associated with the back label glue roller on the line #2 labeler. Employees were exposed to amputation injuries when performing work around the unguarded parts. c) On or about June 27, 2018, the employer did not ensure that employees were protected from the nip points associated with front label glue roller on the line #3 labeler. Employees were exposed to amputation injuries when performing work around the unguarded parts. d) On or about June 27, 2018, the employer did not ensure that employees were protected from the nip points associated with the back label glue roller on the line #3 labeler. Employees were exposed to amputation injuries when performing work around the unguarded parts. e) On or about June 27, 2018, the employer did not ensure that employees were protected from the rotating pallets associated with the back label glue roller on the line #2 labeler.
