A) Canaan Companies on or about June 27, 2018 at the address of 28 North Ontario Street, OH: The employer did not ensure employees were protected from falls while working from a supported scaffold platform. The employee was exposed to a 15ft fall to the level below while performing overhand bricklaying work, near the unguarded end of the scaffold platform.
