29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  Employees engaged in residential roofing operations, using a pneumatic nail gun, at the jobsite at 8014 Clay St.,  Wichita, KS, were exposed to stuck-by hazards in that no eye protection was utilized during operation of the nailer.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  Employees engaged in residential roofing activities the jobsite located at 8014 Clay St., Wichita, KS, were exposed to fall hazards of approx. 9' 3" in that no conventional fall protection systems were in place at the time of the inspection.

29 CFR 1926.1053(b)(16): Portable ladders with structural defects, such as, but not limited to, broken or missing rungs, cleats, or steps, broken or split rails, corroded components, or other faulty or defective components, were not either immediately marked in a manner that readily identifies them as defective, or tagged with "Do Not Use" or similar language, and were not withdrawn from service until repaired.(  Employees engaged in residential roofing activities at the jobsite located at 8014 Clay St., Wichita, KS, were exposed to fall hazards in that the portable ladder that was used to access the roof was shortened with a reciprocating saw and continued to be in service.
