Elm Street, where employees were performing roofing operations while using Hitachi nail guns. Employees were not wearing safety glasses and were exposed to eye injuries.

Elm Street, where employees were not provided with a ladder that did not extend 3-feeet above the landing of the roof and exposing employees to a fall hazard: (a) One (1) Werner 20-feet extension ladder, model number D1220-2 located on the Southeast side of the new building. (b) two (2) Werner 20-feet extension ladders located on the Northwest side of the new building.
