On or about June 27, 2018, at the above addressed jobsite, the portable ladder used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about June 27, 2018, at the above addressed jobsite, employees performing roofing operations were exposed to falls of approximately nine (9) feet six (6) inches and ten (10) feet two (2) inches, created by the presence of unprotected sides. Jose Loaiza dba Loaiza J Construction was previously cited for a violation of this Occupational Safety and Health Standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1219436, citation number 1, item number 1 and was affirmed as a final order on April 4, 2017, with respect to a workplace located at Lot 312 Arthur Drive, Manhattan, IL 60442.
