This was most recently documented at 4902 Birchwood Drive, Bellevue, Nebraska on June 26, 2018 when employees were observed installing weatherproofing materials to a residential structure while working greater than 6 feet above the ground without fall protection.

This was most recently documented at 4902 Birchwood Drive, Bellevue, Nebraska on or about June 26, 2018 when employees were observed using a ladder which was installed with its lower support rails on the peak of a roof.
