29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  The employer is failing to protect employees from fall hazards when conducting roofing operations on residential structures.  This was most recently documented at 4407 Anderson Grove, Bellevue, Nebraska on June 27, 2018 when employees were observed installing weatherproofing materials to the roof of a residential structure while working greater than 6 feet above the ground without fall protection.  The roof eve height was 9'6" with an 8:12 pitch.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access:  The employer is failing to protect employees from fall hazards while climbing ladders to perform roofing operations.  This was most recently observed at 4407 Anderson Grove, Bellevue, Nebraska on June 27, 2018 when a ladder was being used to access a residential structure roof and its side rails did not extend at least 3 feet past the roof eve.  The side rails extended approximately 6" past the roof eve.
