29 CFR 1910.212(a)(1): The employer did not ensure one or more methods of machine guarding was provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about May 22, 2018, employees working at the sort conveyor in the West Building were exposed to caught-in hazards when ingoing nip points were not guarded in the following instances: a.) Employees using the stairs to access the elevated work platform were exposed to caught-in hazards when an in-going nip point between the material conveyor and drum was not guarded. b.) Employees standing at the control box were exposed to caught-in hazards when an in-going nip point between the material conveyor and drum was not guarded.

29 CFR 1910.303(b)(2): The employer did not ensure listed or labeled electrical equipment was used or installed in accordance with instructions included in the listing or labeling. On or about June 8, 2018, employees working in the Truck Shop Building were exposed to electrical hazards from a relocatable electrical power tap used for powering overhead lights. The relocatable power tap was not connected to a permanently installed branch circuit receptacle outlet and was connected in series to another extension cord.
