29 CFR 1926.100(a):  Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  a)  Zamarripa Construction LLC located at 2441 163rd Place, Thornton, CO 80602:  On and preceding June 20, 2018, the employer did not ensure that employees were protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects.  The rigger who was connecting and rigging trusses for the crane operator was working around and under the hoisted load without head protection.  The employees on the roof who were responsible for moving the trusses into place were working under the loads without head protection.  This condition exposed employees to struck-by hazards.

29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a)  Zamarripa Construction LLC located at 2441 163rd Place, Thornton, CO 80602:  On and preceding June 20, 2018, the employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from projectiles to the eyes through the use of pneumatic nail guns.  Employees using and working near pneumatic nail guns were installing trusses without safety glasses.  This condition exposed employees to projectile hazards to the eyes.

29 CFR 1926.501(b)(13): Each employees engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor were employees provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) Zamarripa Construction LLC located at 2441 163rd Place, Thornton, CO 80602: On and preceding June 30, 2018, the employer did not ensure that employees engaged in residential framing activities 6 feet or more above lower levels were protected by guardrail systems, safety net systems, or personal fall arrest systems.  Two employees were installing roof trusses on a two story residential home without the use of fall protection.  This condition exposed employees to fall hazards of up to approximately 28 feet.

29 CFR 1926.1052(c)(1)(ii): Stairways having four or more risers or rising more than 30 inches, whichever is less, were not equipped with a stairrail system along each unprotected side or edge:  a) Zamarripa Construction LLC located at 2441 163rd Place, Thornton, CO 80602: On and preceding June 30, 2018, the employer did not ensure that stairways with four or more risers and unprotected sides were protected by a stairrail system. The staircases in a two story residential home with a basement were used to access the upper and lower levels by the framers.  All four staircases were unprotected on one side with 7 or more risers.  This condition exposed employees to fall hazards of up to approximately 10 feet.

29 CFR 1926.1412(d)(1): A competent person did not begin a visual inspection prior to each shift the equipment will be used, which must be completed before or during that shift. The inspection must consist of observation for apparent deficiencies. Taking apart equipment components and booming down is not required as part of this inspection unless the results of the visual inspection or trial operation indicate that further investigation necessitating taking apart equipment components or booming down is needed. Determinations made in conducting the inspection must be reassessed in light of observations made during operation. A National Series 4 truck mounted telescoping crane was being utilized to hoist trusses in place.  The operator/owner of the crane left the controls and walked to a different job site to retrieve a different piece of equipment.  Barricades or caution lines, and notices, were not erected to prevent all employees from entering the fall zone beneath the truss which was being held by the crane.  This condition exposed employees to struck-by hazards.
