29 CFR 1926.304(h)(1): Each circular crosscut table saw was not guarded by a hood that which shall completely enclose that portion of the saw above the table and the material being cut: a) During construction renovation of the 7 story building a table saw was being used in the cellar with an unguarded blade. Employees were exposed to amputation when using the saw. Location: 830 Flatbush Ave, Brooklyn NY On or about 8/1/2018 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.

Employees were exposed to electric shock hazard; --- 3fl utility closet; live wire was sticking out of the wall with live tip exposed, 110v; Location: 830 Flatbush Ave, Brooklyn NY On or about 8/1/2018 NOTE: BECAUSE ABATEMENT OF THIS VIOLATION IS ALREADY DOCUMENTED IN THE CASEFILE, THE EMPLOYER NEED NOT SUBMIT CERTIFICATION NOR DOCUMENTATION OF ABATEMENT OF THIS VIOLATION AS NORMALLY REQUIRED BY 29 CFR 1903.19.
