29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii): a) On or about 6/28/18 at a bridge replacement project on County Road 6 in Leon, New York; employees were using a bracket scaffold while constructing concrete forms. The work platform was only a single 2"x12" plank wide with approximately 20" to 24" of open space for additional plank and toe boards. NO ABATEMENT CERTIFICATION REQUIRED

29 CFR 1926.451(e)(1): A means of access for scaffold platforms more than 2 feet (0.6 m) above or below a point of access, such as portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: a) On or about 6/28/18 at a bridge replacement project on County Road 6 in Leon, New York; employees were using a bracket scaffold while constructing concrete forms. The ladder to access the platform was leaning on the forms below the platform. Employees had to climb around the scaffold platform to access the scaffold. NO ABATEMENT CERTIFICATION REQUIRED
