29 CFR 1910.29(b): Guardrail systems. The employer must ensure guardrail systems meet the following requirements: 29 CFR 1910.29(b)(2): Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high: a. On or about July 02, 2018, in the AS/RS Area, maintenance employees were exposed to approximately 20 foot falls while unjamming boxes near the edges of the racking system platforms. Steel cables were in place for top rails but no midrails were installed to ensure employees were protected against falls on the edges of the platforms.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by this standard, live parts of electric equipment operating at 50 volts or more shall be guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the following means: a. On or about July 02, 2018, in the Sorting Area, the Allen-Bradley variable frequency drive for the LPA Sorter did not have its dead front cover in place, exposing employees to 480V electrical components. The cover had been removed for troubleshooting and had been replaced with a piece of paper and packaging tape.
