29 CFR 1910.23(b):  General requirements for all ladders. The employer must ensure:     29 CFR 1910.23(b)(8): Ladders are used only for the purposes for which they were designed:    a. On or about June 26, 2018, located at the southwest corner of the foundry, an employee climbed a Werner stepladder, model number PT6002, in order to access the mold transfer while the ladder did not extend at least three feet above the upper landing surface; thereby exposing the employee to a maximum fall hazard of approximately four feet three inches.

Sancast, Inc. a Subsidiary of Wabtec was previously cited for a violation of its equivalent standard 1910.147(c)(4)(ii), which was contained in OSHA inspection number 1163018, citation number 2, item number 3 and was affirmed as a final order on February 10, 2017, with respect to a workplace located at 535 Clow Lane Coshocton, Ohio 43812.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:  a. On or about June 26, 2018, located southwest corner of the foundry, employees were exposed to machine hazards associated with moving parts of a hydraulic mold pusher during the setup activity of a headboard change on the Hunter Mold Machine. The employer failed to implement energy control application steps as the interconnected mold transfer line was not shut down or turned off to perform the work [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine hazards.  Sancast, Inc. a Subsidiary of Wabtec was previously cited for a violation of this Occupational Safety and Health standard  which was contained in OSHA inspection number 1163018, citation number 1, item number 2a and was affirmed as a final order on February 10, 2017, with respect to a workplace located at 535 Clow Lane Coshocton, Ohio 43812.

29 CFR 1910.147(f)(3)(ii)(D): Each authorized employee shall affix a personal lockout or tagout device to the group lockout device, group lockbox, or comparable mechanism when he or she begins work, and shall remove those devices when he or she stops working on the machine or equipment being serviced or maintained.    a. On or about June 26, 2018, located at the southwest corner of the foundry , three employees were involved in a group lockout/tagout activity in performing a headboard change on a Hunter Mold Machine. The employees involved in the headboard change did not have their locks attached to all energy isolating devices, thereby exposing the employees to a caught-in hazard.    Sancast, Inc. a Subsidiary of Wabtec was previously cited for a violation of this Occupational Safety and Health standard  which was contained in OSHA inspection number 1163018, citation number 1, item number 2b and was affirmed as a final order on February 10, 2017, with respect to a workplace located at 535 Clow Lane Coshocton, Ohio 43812.
