29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that are 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about July 2, 2018, during trench and excavation activities, the employer did not ensure that a safe means of egress was provided in the trench/excavation in that a ladder was placed beyond of a shoring jack in such a position that it forced an employee to go either over or under it to reach the ladder for egress, which impeded the employee's ability to get out of trench/excavation quickly and safely.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could result in possible cave-ins, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about July 2, 2018, during trench and excavation activities, the employer did not ensure that a competent person performed an appropriate inspection of the trench/excavation. Deficiencies noted during the inspection included, but were not limited to, lack of a safe means of egress, damaged materials being used as shoring components in some areas of the trench/excavation and no cave in protection used in some areas of the trench/excavation, exposing an employee to a cave in hazard of up to at least approximately 8 1/2 feet or greater in depth.

29 CFR 1926.652(d)(1): Materials and equipment used for protective systems were not free from damage or defects that might impair their proper function: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about July 2, 2018, during trench and excavation activities, the employer did not ensure that plywood sheets used for cave in protection in a trench/excavation measured to be up to at least approximately 8 1/2 feet or greater in depth was appropriate to use in that some wood had visible vertical cracks in it, holes had been cut into sheets, at least one sheet was twisted/warped into place and one sheet had paneling layers that were observed to have been coming apart.
