29 CFR 1926.20(b)(2): The program did not provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about August 28, 2018, the employer did not ensure that an appropriate system was implemented at the site to have safety inspections conducted in order to identify and correct safety and health hazards. Deficiencies noted at the site included, but were not limited to, fall hazards at window openings or on scaffolding, temporary wiring electrical hazards such as improper location of splicing, exposure to unnecessary contact or being subjected to sharp edges, exposure to internal wiring on temporary lighting hung at about 4 feet from the floor, fuel dispensing systems not properly protected from contact, temporary lighting bulbs not protected from contact by guards, damaged extension cords and unsafe ladders used at the site for a variety of jobs.
