29 CFR 1926.451(b)(3): The front edge of all platforms were permitted to be more than 14 inches (36 cm) from the face of the work, and guardrail systems were not erected along the front edge and/or personal fall arrest systems were not used in accordance with paragraph (g) of this section to protect employees from falling: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about July 2, 2018, during siding activities, the employer did not ensure that scaffold platform use was compliant, in that it was determined to be approximately 18 inches from the side of the building and no form of fall protection was in use, exposing an employee to a fall hazard of up to at least approximately 13 feet.

29 CFR 1926.454(c): When the employer had reason to believe that an employee lacked the skill or understanding needed for safe work involving the erection, use or dismantling of scaffolds, the employer did not retrain each such employee so that the requisite proficiency was regained: a) Job site, 6434 Station Street, Pittsburgh, PA: On or about July 2, 2018, during siding activities, the employer did not ensure that the scaffolds were properly used. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection and not ensuring the that scaffold platform was within 14 inches of the side of the building, both of which exposed an employee to a fall hazard of up to approximately 13 feet.
