Jobsite: The employee working from the pump jack scaffold was not protected from the hazard of falling approximately 18 feet to the ground by the use of appropriate fall protection systems and/or devices.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards. Jobsite: The employee exposed to a fall hazard approximately 18 feet to the ground while working from the pump jack scaffold was not trained by a qualified person in the appropriate subject matter to recognize the hazards and the procedures to use to control or minimize those hazards.
