29 CFR 1926.451(g)(1)(vii): Each employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.    Jobsite: The employee working from the pump jack scaffold was not protected from the hazard of falling approximately 18 feet to the ground by the use of appropriate fall protection systems and/or devices.

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards.    Jobsite: The employee exposed to a fall hazard approximately 18 feet to the ground while working from the pump jack scaffold was not trained by a qualified person in the appropriate subject matter to recognize the hazards and the procedures to use to control or minimize those hazards.

29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.      Jobsite: The poles of the aluminum pump jack scaffold from which an employee was working were not secured at the bottom and susceptible to being displaced.
