29 CFR 1926.451(g)(1)(vi): Each employee performing overhand bricklaying operations from a supported scaffold was not protected from falling from all open sides and ends of the scaffold (except at the side next to the wall being laid) by the use of a personal fall arrest system or guardrail system (with minimum 200 pound toprail capacity). a) On or about 06/30/2018, at 2600 Technology Drive, Orlando, Florida, employees in the process of bricklaying activities were exposed to a 15'8" fall hazard, in that a guardrail system was not installed on the 3rd level of the side brackets/work platforms of the scaffolding system. b) On or about 06/30/2018, at 2600 Technology Drive, Orlando, Florida, employees in the process of bricklaying activities were exposed to a 22 foot fall hazard, in that a guardrail system was not installed on the 4th level of the side brackets/work platforms of the scaffolding system.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: a) On or about 06/30/2018, at 2600 Technology Drive, Orlando, Florida, an employee fell from a scaffolding system causing trauma to the head and was hospitalized. The employer was aware of the in-patient hospitalization on Saturday, 06/30/18. The employer did not notify OSHA of the hospitalization.
