29 CFR 1926.502(d)(15): Anchorage points used for attachment of personal fall arrest equipment were not independent of any anchorage point being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee a. On or about July 2, 2018, two roofing employees that were exposed to 12-14 feet fall hazards set up fall protection systems with anchor plates that were not correctly installed per manufacturer instructions in that there were an insufficient number of fasteners installed, the plates were damaged, the plates were not flat against the roof surface, and the fasteners were not the correct type, reducing the pull strength of the anchor.
