OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   a)  Worksite - On or about July 5, 2018, employees performing work activities in the warehouse were exposed to struck-by and crushing hazards because a damaged racking system was being used to store and retrieve material.  The employer did not ensure that the racking system was inspected and the damaged sections were repaired or replaced.  Abatement Note:  One feasible method to correct the hazard includes, but is not limited to: Follow MH16.1- 2008, ANSI-RMI Specification for the Design, Testing and Utilization of Industrial Steel Storage Racks, Section 1.4.9 Rack Damage, which states "Upon any visible damage, the pertinent portions of the rack shall be unloaded immediately by the user until the damaged portion is repaired or replaced".
