On or about July 03, 2018, on the residential construction site where employees were exposed to a fall hazard greater than 6 feet when performing siding work from the roof of a residential structure being constructed without a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On or about July 03, 2018, at the south side of the residential construction site where an employee was exposed to a fall hazard when using an extension ladder to access a roof without extending the ladder side rails at least three feet above the roof landing.
