29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access.  A- Pump Jack Scaffold: Employees were engaged in siding work from a pump jack scaffold in which the scaffold work platform was not provided a means of scaffold access exposing workers to fall hazards of approximately 16ft to the concrete below. Condition noted on or about 07/02/2018

29 CFR 1926.451(f)(6): Scaffolds were erected, used, dismantled, altered, or moved such that they, or any conductive material handled on them, could have come closer than the minimum distances established for specific voltages as set forth in the table in subparagraph (f)(6).Note:  Specify distances as indicated in table in paragraph (f)(6) of this section.  A- Pump Jack Scaffold: Employees engaged in siding work from a pump jack scaffold were exposed to electrocution hazards as an energized secondary power line had not been de-energized prior to the start of work. 29 CFR 1926.452(j)(2): Poles were not secured to the structure by rigid triangular bracing or equivalent at the bottom, top, and other points as necessary.  A- Pump Jack Scaffold: Employees were engaged in siding work from a pump jack scaffold in which the supporting legs were not provided triangular bracing at the base exposing workers to scaffold collapse. Condition noted on or about 07/02/2018
