29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a) 1013 Hanover Avenue, Allentown, PA, Worksite, Roof - The employer does not ensure that appropriate fall protection equipment or methods are in use. A specific instance of this continuing violation was most recently observed on or about July 5, 2018 when employees were performing residential roofing activities while working without fall protection at the unprotected edges of a steeply pitched roof, approximately 12 to 25 feet above a lower level. The employer's ongoing obligation is to ensure that fall protection equipment and/or methods are in use when employees are exposed to fall hazards.  AJ Kerekes Roofing was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1189342, citation number 1, item number 1 and was affirmed as a final order on December 5, 2016, with respect to a workplace located at 1020 N. 19th Street, Allentown, Pennsylvania.  AJ Kerekes Roofing was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1077933, citation number 1, item number 1 and was affirmed as a final order on August 28. 2015, with respect to a workplace located at 27 Salor Drive, Coplay, Pennsylvania.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a) 1013 Hanover Avenue, Allentown, PA, Worksite, Roof - on or about July 5, 2018, employees were performing residential roofing work while accessing the roof areas by utilizing an extendable ladder system which was not extended at least three feet above the upper landing surface.    AJ Kerekes Roofing, LLC was previously cited for a violation of this occupational safety and health standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 1189342, citation number 1, item number 2 and was affirmed as a final order on December 5, 2016, with respect to a workplace located at 1020 N. 19th Street, Allentown, Pennsylvania.
