29 CFR 1910.178(p)(1): Powered industrial truck found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition: Creative Logistic Services, LLC/Monroe Township, NJ- Warehouse Area: Employer did not remove from service forklifts which had visible defects: a) Toyota propane powered forklift (#2) had damaged light and the top rung on the forklift cage was broken. Additionally, the concentration levels of carbon monoxide (CO) gas created by the forklift were measured to be 122 parts per million (ppm). b) Toyota propane powered forklift (#4) had broken propane latch. Condition observed on or about July 5, 2018.
