29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a)Global Polymer Industries, Inc. at 1001 Southeast 12th Street, Madison, SD 57042: On and about and at times prior to July 5, 2018, the employer did not ensure employees were protected from pinch-point hazards in a that a means of effective machine guarding was not utilized to protect employees from pinch-points created by upward and downward cycling of the ejector on Mold 10" WE 4110. This condition exposed employees to pinch-point hazards that resulted in an employee fingertip amputation.
