Knox Ct., Denver, CO 80204:  On and before April 16, 2018, the exposing employer did not ensure that modifications or additions which affect the capacity or safe operation of the equipment were made with the manufacturer's written approval.  The employer allowed the use of a rough-terrain forklift, Genie Model GTH 844, to lift a bundle of rebar with nylon slings attached to one tine.  Lifting of a load in this manner affected the center of gravity, carrying capacity, and the security of the load.  This condition resulted in employee exposure to being struck-by falling and swinging loads.
