29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): At the residential construction site located at 1812 NE 89th Place in Liberty, Missouri: Employees engaged in residential framing activities, were exposed to fall hazards of approximately 14 feet from the top plate of the first level of the residence to the interior of the structure in that fall protection was not utilized.

29 CFR 1926.501(c): When employees were exposed to falling objects the employer did not require employees to wear hard hats and did not implement measures to prevent objects from falling from higher levels as set forth in (c)(1)-(3) of this section: At the residential construction site located at 1812 NE 89th Place in Liberty, Missouri: Employees working below employees working at heights were exposed to struck-by hazards in that employees at the lower levels were not provided hard hats.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: At the residential construction site located at 1812 NE 89th Place in Liberty, Missouri: Employees were exposed to fall hazards. Roofers were observed accessing and egressing the roof by way of a portable ladder that did not extend 3 feet past landing. Framers were exposed to a fall distance of greater than 14 feet eave to ground without use of fall protection.
