Location: a) On the vehicle mounted aerial lift basket at the jobsite: On or about 07/02/18, employee was removing flags from the gas station while working on an aerial lift without wearing a personal fall arrest or travel restraint system, and was exposed to falling approximately 13 feet to the lower level.
